Support Coordination is a ‘Capacity building’ NDIS funded support. It is there to help with the implementation of all NDIS supports within a participant's plan, and helps participants with connections to mainstream and community supports.
Support Coordinators work with participants, their families and their current support providers. Support Coordinators will assist you to link with a range of services and supports to increase your capacity, independence and participate in your community. Support Coordinators will help you to understand your NDIS plan and work with you to ensure a mix of supports are used to increase your capacity and independence. At Sunshine we value each individuals choice and control and work with NDIS participants to build their independence and decision making capacity.
Level 2 - Coordination of Supports: This support assists participants to understand their NDIS plan and engage with supports that will meet their individual needs. Support Coordinators assist participants to increase their capacity, maintain relationships, access appropriate services and manage service provision.
Level 3 - Specialist Support Coordination: Specialist Support Coordination is delivered by a qualified and experienced practitioner to address points of crisis, complex needs, or high level risks. These funds are used to address barriers that are impacting a participants access to appropriate supports. Specialist Support Coordinators assist participants to reduce complexity in their environment, and to overcome barriers when connecting to mainstream and funded supports.
Psychosocial Recovery Coach: This support is included under the Support Coordination budget. It provides assistance for participants to build capacity and resilience through strong and respectful relationships. Psychosocial Recovery Coaches promotes engagement through periods of increased support needs due to the episodic nature of mental illness. Recovery coaches work collaboratively with participants, families, carers and other services to identify, plan, design and coordinate NDIS supports.
Individualised Living Options - Exploration and Design is utilised from the Core budget to support participants looking at individualised living options.
· Explore the vision of the participant for their future home and help all involved to understand the full range of options as to how a participant can be supported in the community;
· Gain an understanding of a participant’s strengths, with a focus on what they can do, and an understanding of how informal, formal and community supports will play a role in the participant’s life, and build on this to design the individual support package;
· Identify strategies to help the participant choose where to live, who to live with and how they are supported;
· Find out what is possible and assess risks in the context of the participant’s preferred way of living, and explore if there could be other options that might suit better; and
· Design and make decisions with the participant and others about the options.
Supporting participants to achieve their goals, connect with their community, increase independence and develop their capacity.
